In order to start the plan, daily resource use had to be accounted for at Goncalo's Health Center. Opportunities for growth, discussed in a multidisciplinary team meeting, were later implemented. To effectively reach the community with our intervention, the local government offered valuable cooperation.
A considerable reduction in resource expenditure was observed, with a primary focus on the decrease in paper use. The previous system of waste management, devoid of separation and recycling, has been transformed by this program, which initiated these practices. Within Goncalo's community, the Parish Council building, Health Center, and School Center saw the implementation of this change, which involved promoting health education.

A substantial amount of research now supports the beneficial role of self-blood pressure monitoring (SBPM) in controlling hypertension among patients. Not only is this method budget-friendly and well-tolerated, but it also has proven to be a better indicator of end-organ damage compared to the usual office blood pressure monitoring.
